不好意思我的問題沒有表達清楚。
想問的是"第一次開立慢性連續處方簽"時英文該怎麼描述？
"refill medication"應該是指第二第三次慢簽或是照上次拿藥時用的主述......

要打在病歷上就設定代碼就好啦
"chronic illness prescription refill slip "
健保局上面寫的，這大概是健保局自己發明的吧

The NHI medical care provider handling chronic illness (as shown in Annex 1) patients who need the same prescription medication for a long time shall issue a chronic illness prescription refill slip except the medication is grade-one or grade-two controlled drug defined in the Statute for the Control of Controlled Substances.
Only one chronic illness prescription refill slip could be issued for a chronic illness.

就我經驗國外大概沒有這種表達方式.
因為處分就是處方,無論急慢性.
譬如:處方寫法
CELEBRAX 200 MG 1 QD X30
REFILL X2
這樣就可以領3次.
所以你的描述大概是 PT IS PROVIDED A PRESCRIPTION FOR ARTHRITIS(譬如關節炎) WITH 1 MONTH SUPPLY & 2 TIMES REFILL
